Awful book! Do NOT purchase!!

This book is AWFUL! The way it is organized does not make it user friendly. Any time I have ever used it to help write a care plan in class, the teacher has told me the my outcomes/interventions are wrong when I got them straight from the book. I would think it was me and not the book, but when I borrow a friend's book and use that for my care plans I get them right. Also, when you look something up in the index (say, dehydration for example) it does not give you a variety of nursing diagnoses. Instead, it gives you one (in this case, risk for imbalanced temperature). There are many diagnoses related to dehydration, but unless you read through every diagnois you are not going to be able to find it. Also, it should have a list of diagnoses so you can find the one you want, and then look up the related outcomes, interventions, etc. This book was one of the most disappointing purchases I have ever made. I would have given it 0 stars if I could have.

Great Nursing Plan Guide book

Has tons of nursing diagnosis. Hard to navigate initially but great format once you understand how to search. Intervention and implementations not super detail because it is just a pocket but long enough to understand. Like that it has an intervention for each nursing process step. Recommend this book to nurses that create plans on a routine basis and for nursing students.

Awesome pocket guide

This pocket guide is very handy to have around. I would definetly recommend having a bigger more thorough Nursing Diagnosis book available but this one was a great tool to have in your arsenal. It gives you a few key interventions (assess, implement, teach). I like looking in this one first to see which ones they list because they can sometimes be stated much more simply than in the bigger books. It does give you rationales but not for all of them. All in all I'm happy I bought it. It has come in handy more than a few times.
