In recent years, novel pharmaceutical therapies have given rise to both new hopes for recovery and renewed fears about drug diversion and abuse. In The Clinic and Elsewhere, Todd Meyers looks at the problems of meaning caused by drug dependency and appraises the changing terms of medical intervention today.

By following a group of adolescents from the time they enter drug rehabilitation treatment through their reentry into the outside world-the clinic, their homes and neighborhoods, and other institutional settings-Meyers traces patterns of life that become mediated by pharmaceutical intervention. His focus is not on the drug economy but rather on the therapeutic economy, where new markets, transactions of care, and highly porous conceptions of success and failure come together to shape addiction and recovery. The book is at once a meditative work of anthropology, a demonstration of the theoretical and methodological limits of medical research, and a forceful intervention into the philosophy of therapeutics at the level of the individual.

A powerful, insightful and searching look at the techniques and technologies of drug treatment. A combination of a complex philosophical discussion of science and medicine with harrowing and beautiful portraits of adolescent patients.

Good, but missing some important information...

I enjoyed the book. The work is definitely valuable.

However, I was left with questions - questions that he must've been able to answer. Unfortunately, I was left feeling he didn't want to give those answers, and hid his responsibility to provide them underneath many philosophical detours.

The most important question, to me, is - were the 12 patients he followed actually helped by their buprenorphine treatment odyssey?

He mentions that he followed 12 patients, but going back through the book again, I could only identify 8 or 9 that he actually discussed. Of those he discussed, it seems that only one successfully stopped using opiates for an extended period of time, but was murdered shortly thereafter due to his life as a drug dealer.

Other than that, it seems like everyone continued to struggle. Perhaps the 3 or 4 patients he didn't discuss actually found some long term change in their substance use habits? I feel like that would've been notable though, in contrast to the fate of the others. The apparent omission of these results has me puzzled. All I can come up with as a reason for this omission is that he found something so contrary to his expectations that he had to look the other way. Even if the patients quickly disappeared, he was at least privy to this fact, and could have disclosed it.

Or maybe he did discuss those patients, and I missed it somehow. I scoured the book several times though, and couldn't find it. I'm giving 3 stars, because I think readers deserved some sort of summary of the outcomes he witnessed. Without this, the heavy philosophizing, while interesting, seems like a purposeful distraction. He essentially says he doesn't want to make his subjects into statistics or use them to declare the treatment a success or failure, but this feels like a cop-out. He had the unique experience of following these study subjects beyond the 12 week window of most buprenorphine trials, he had the ability to tell us more, and for some he reason, he chose not to.

Whatever the intentions of the author, and regardless of the missing information, this book is still a challenge to buprenorphine's status as a "wonder drug" for opiate addiction.
